By scheduling with us you acknowledge and agree to the following:
- If you are using insurance treatment must be medically necessary, preventative and maintenance care are not a covered service and will not be billed through your insurance.
- Any chronic conditions that are believed to be medically necessary must have a note from Regence with a name of the representative spoken with, that states that they are cleared for treatment.
- Any medical massage must be scheduled by phone.
- Online scheduling available for Wellness treatment only.
- Scheduled appointments are the core of our business. Therefore we require a minimum of 48 hours notice for cancellation or change.
- Last minute calls and no shows will be charged 100% of the scheduled service.
- Reminder emails are a courtesy. You are responsible for knowing your appointment day and time. Please be responsible for your appointments.
- We request that you show up 10 minutes early for your appointment to unwind and fill out any necessary paperwork.
- Please remember to turn off your cell phone or leave it in your car.
- Please try to be hydrated for your appointment. Drink plenty of water.
- You are responsible for knowing your coverage benefits and prescription refills regarding massage therapy
